Patient Forms

 Welcome to Mediwise! To ensure a smooth and efficient process for all patients undergoing dialysis treatment, it is essential to have the right documentation ready. Below, we have outlined the key forms and requirements based on the type of visitor.


For First-Time Visitors

If this is your first time visiting Mediwise for dialysis treatment, please ensure you have the following forms:

  1. Personal Identification: A government-issued ID or passport for verification.
  2. Referral Letter: A signed referral letter from your nephrologist or primary care physician.
  3. Medical History Report: A detailed report from your nephrologist, including your medical history and dialysis requirements.
  4. Prescription: A current prescription for dialysis treatment, usually issued by your nephrologist.
  5. Lab Test Results: Results from recent laboratory tests, including:
    • Kidney function tests (e.g., serum creatinine, GFR)
    • Complete blood count (CBC)
    • Viral markers (e.g., Hepatitis B, Hepatitis C, HIV)
    • Other relevant tests recommended by your doctor • Note: Lab results must be no older than two weeks. If your results exceed this timeframe, you will need to retake the necessary tests.
  6. Insurance Information: Local insurance documentation for billing purposes. International insurance is not accepted.
  7. Authorization Forms: Completed and signed consent forms for dialysis treatment, which can be obtained on arrival.

For Transferring patients

For patients returning to Mediwise for ongoing dialysis treatment, please bring the following:

  1. Updated Medical Reports: Any new reports or updates from your nephrologist since your last visit.
  2. Current Prescription: An updated prescription for dialysis treatment from your nephrologist.
  3. Current Lab Results: Ensure lab results, about 1 month old. Tests may include kidney function, CBC, and viral markers.
  4. Insurance Verification: An updated copy of your local insurance card or proof of coverage, unless you are paying in cash.
  5. Previous Visit Records: Any documentation from your prior sessions at non-Mediwise units, if not already on file.

For Foreign Visitors

We welcome foreign visitors seeking dialysis treatment. Please note the following requirements and policies:

  1. Payment Policy: International insurance is not covered. Be prepared to pay for your treatment in cash. You can collect receipts/invoices and submit your insurance claims to your provider.
  2. Required Documentation:
    • A valid passport for identification.
    • Recent medical history and a referral letter from your nephrologist/doctor
    • A current prescription for dialysis treatment, usually issued by your nephrologist. (May it include dialysis authorization.)
    • Lab test results , plus or minus 1 month old. Tests should include kidney function tests, CBC, and viral markers, etc.
  3. Advance Booking for a Initial consultation: It is strongly recommended to book your consultation first, and we can then schedule your sessions in available slots. In emergencies, you will be dialyzed as soon as you arrive.
